Evidence from two prospective cohorts shows that variability in glucose disposal rate over time—not just average insulin sensitivity—predicts progression to frailty in older adults. The estimated glucose disposal rate (eGDR) serves as a practical marker of how efficiently the body handles blood sugar, with instability reflecting metabolic dysregulation. People whose insulin sensitivity fluctuated more widely faced higher risk of developing frailty, even after accounting for average glucose control and other confounders. This matters because frailty represents a critical vulnerability state that predicts falls, disability, and mortality. The findings suggest that metabolic stability, not just metabolic health at any single timepoint, may be crucial for maintaining resilience with age. Clinically, this could shift focus toward minimizing glucose variability through consistent diet, exercise, and medication adherence rather than simply targeting average blood sugar levels. The replication across two independent cohorts strengthens confidence, though interventional studies are needed to confirm whether stabilizing glucose metabolism can actually prevent frailty.
