Rapamycin trial shows increased brain blood flow in middle-aged APOE4 carriers

A small but significant human trial published in the Journal of Cerebral Blood Flow and Metabolism demonstrates that rapamycin, the most studied longevity drug, increases blood flow to the brain in middle-aged adults carrying APOE4—the strongest genetic risk factor for late-onset Alzheimer's disease. This single-arm pilot study also found favorable changes in metabolic markers, inflammatory profiles, and even gut microbiome composition. The cerebral blood flow finding is particularly relevant because reduced brain perfusion precedes cognitive decline and appears decades before dementia symptoms. While this was a small, uncontrolled trial without a placebo group, the results align with animal studies showing rapamycin's neuroprotective effects and add human evidence to the case for mTOR inhibition in brain aging. APOE4 carriers face 3-15 times higher Alzheimer's risk, making this a high-priority population for prevention strategies. The multi-system benefits—vascular, metabolic, immune, and microbiome—suggest rapamycin may work through several complementary mechanisms. Larger randomized controlled trials are needed, but this early human data strengthens the rationale for rapamycin as a potential Alzheimer's prevention tool.
