Metformin reduces age-related heart fibrosis by boosting fat metabolism

Researchers found that metformin inhibits the transformation of cardiac fibroblasts into myofibroblasts—the cells responsible for heart scarring and stiffness—by ramping up fatty acid β-oxidation. Age-related cardiac fibrosis is a major contributor to heart failure in older adults, and metformin users have long shown better cardiovascular outcomes in observational studies. This work, conducted in cell culture, suggests a specific metabolic mechanism: by shifting fibroblast energy production toward fat burning, metformin may prevent the metabolic rewiring that drives fibrosis. The findings are preliminary and based on in vitro models, so we should be cautious about over-interpreting implications for human hearts. Still, they add to a growing body of mechanistic evidence that metformin's benefits extend beyond glucose control. If confirmed in animal and human studies, this could help explain why metformin is increasingly seen as a candidate geroprotective drug with multi-organ benefits.
